Safety
As the course name suggests, safety is the primary objective of this course. We only recommend treating ASA I and II patients with IV moderate sedation. ASA III patients should be referred or treated in your office with an anesthesiologist present. Patient selection is critical when sedating patients.

Titration
Titration makes IV sedation unique. Sedation is a continuum from awake to general anesthesia. Fentanyl and midazolam are added slowly in small increments until the patient is moderately sedated.
Dr. Wayland
Dr. John Wayland is the course director and founder of Bay Area IV Wisdom. He was trained in IV sedation by Dr. Stanley Malamed in 1982. Dr. Malamed is an anesthesia icon and the author of “SEDATION: A Guide to Patient Management.”
Dr. Wayland became certified in IV sedation in 1983. He has been a Dental Board of California office sedation evaluator for more than 20 years.

Safe moderate sedation is our goal. Moderately sedated patients are responsive to light tactile stimulation or verbal commands. This is accomplished with titration, the slow administration of fentanyl and midazolam (Versed). These drugs, fentanyl and midazolam, are reversible. This is not true for propofol, dexmedetomadine, clonidine and other drugs taught in many courses. Bay Area IV Wisdom uses minimal drugs to reach moderate levels of sedation.

Studies show that the surgical removal of third molars is the most feared of all dental procedures. Every patient is very anxious about this procedure. However, third molar surgery can be completed with no pain and no memory when using IV moderate sedation and local anesthesia. Patients always have many questions from family and friends after the procedure. Their answer is always the same, “It was better than I expected; I don’t remember anything.” Anyone hearing this wants you to be their dentist.

We start with practice on mannequin arms. This builds confidence. We do venipuncture with the dentist partners on a volunteer basis. You will be trained before doing venipuncture on your patients.

The classical moderate IV sedation drugs used in most hospitals today for a colonoscopy are fentanyl and Versed. This is a safe combination. Fentanyl produces euphoria and analgesia, Versed (midazolam) produces sedation and amnesia. This is a classical drug combination that is safe and predictable.

Every participating dentist will receive a free copy of Dr. Stanley Malamed’s book “SEDATION: A Guide to Patient Management” As a bonus, you will also receive a free copy “Impacted Third Molars,” published by Wiley and written by Dr. John Wayland. This book contains two chapters dedicated to sedation.
- Six days — No airfare or hotel for many participants
This course is completed in six days, and you will receive 80 CE hours (20 hours online). You do not need to make multiple trips to our location in San Jose. Most participants can drive to our course location in Los Gatos, California, which eliminates car rental and Uber, saving you money. Everything is completed in six days following the recommendations and guidelines of the ADA and California Dental Board.

Maximum class size is 12 dentists. Ratio of faculty to patient is one to one so that you can receive hands-on training with your instructor.

John Wayland, DDS, FAGD, MaCSD
Dr. Wayland is the course director and founder of Bay Area IV Wisdom and past founder of Western Surgical and Sedation. He has been teaching IV Sedation since 2011.

Betty Yee, Course Coordinator
Betty Yee is our course coordinator. She has the answers to all of your questions. Ms. Yee worked in the technology and software industry for 20 years. During this time, she held many management positions including project manager, development manager and IT/MIS manager. She has worked with many Fortune 100 companies to develop software.

Linda Centore, RN, PhD
Dr. Linda Centore is a psychologist and primary care nurse practitioner with over 30 years of experience in behavioral health. She has been a faculty member of the University of California, San Francisco School of Dentistry for more than 35 years, where she teaches dental students how to work with fearful patients. Dr. Centore is the chair for the division of Behavioral Sciences and Community Dental Education.

Tim Fagin, CPR/ACLS/ECG
Tim Fagin has been a paramedic for many years. He is currently the lead Instructor at Heartshare Training Services Inc. teaching CPR, ACLS and ECG interpretation.

Tom Lenhart, DMD, Anesthesiologist
Dr. Thomas E. Lenhart is a board-certified dental anesthesiologist licensed in California He has been providing oral, conscious sedation and general anesthesia for dentists and physicians for over 30 years.

Matthew Diercks, DDS
Dr. Diercks received his IV conscious sedation permit from the Dental Board of California in 2014 and has been removing impacted third molars with IV sedation twenty times a week since then. He is a California Dental Board office sedation evaluator. The clinical portion of Bay Area IV Wisdom is taught in his Los Gatos, California, office.

Vincent Vella, DDS, DABOI
Dr. Vella has been a licensed dentist in Rochester, New York, since 1969. He is a diplomate of the American Board of Oral Implantology and is recognized as an implant specialist. Dr. Vella has permits in oral and IV sedation.

Our IV sedation course begins Tuesday and ends Sunday at 5 p.m. Safe IV sedation follows the 2016 American Dental Association guidelines for teaching pain control and sedation to dentists and dental students. The course will cover the objectives and content recommended by the ADA and the California Dental Board.
COURSE OBJECTIVES
- List and discuss the advantages and disadvantages of moderate sedation.
- Discuss the prevention, recognition and management of complications associated with moderate sedation.
- Administer moderate sedation to 20 patients in a clinical setting in a safe and effective manner.
- Discuss the abuse potential, occupational hazards and other untoward effects of the agents utilized to achieve moderate sedation.
- Describe and demonstrate the technique of intravenous access, intramuscular injection and other parenteral techniques.
- Discuss the pharmacology of the drug(s) selected for administration.
- Discuss the precautions, indications, contraindications and adverse reactions associated with the drug(s) selected.
- Administer the selected drug(s) to 20 dental patients in a clinical setting in a safe and effective manner.
- List the complications associated with techniques of moderate sedation.
- Describe a protocol for management of emergencies in the dental office and list and discuss the emergency drugs and equipment required for the prevention and management of emergency situations.
- Discuss principles of advanced cardiac life support or an appropriate dental sedation/ anesthesia emergency course.
- Demonstrate the ability to manage emergency situations.
- Demonstrate the ability to diagnose and treat emergencies related to the next deeper level of anesthesia than intended.
COURSE CONTENT
- Historical, philosophical and psychological aspects of anxiety and pain control.
- Patient evaluation and selection through review of medical history taking, physical diagnosis and psychological considerations.
- Use of patient history and examination for ASA classification, risk assessment and preprocedural fasting instructions.
- Definitions and descriptions of physiological and psychological aspects of anxiety and pain.
- Description of the sedation anesthesia continuum, with special emphasis on the distinction between the conscious and the unconscious state.
- Review of adult respiratory and circulatory physiology and related anatomy.
- Pharmacology of local anesthetics and agents used in moderate sedation, including drug interactions and contraindications.
- Indications and contraindications for use of moderate sedation.
- Review of dental procedures possible under moderate sedation.
- Patient monitoring using observation and monitoring equipment, with particular attention to vital signs, ventilation/breathing and reflexes related to consciousness.
- Maintaining proper records with accurate chart entries recording medical history, physical examination, informed consent, time-oriented anesthesia record (including the name of all drugs administered including local anesthetics), doses, and monitored physiological parameters.
- Prevention, recognition and management of complications and emergencies.
- Description, maintenance and use of moderate sedation monitors and equipment.
- Discussion of abuse potential.
- Intravenous access: anatomy, equipment and technique.
- Prevention, recognition and management of complications of venipuncture and other parenteral techniques.
- Description and rationale for the technique to be employed.
- Prevention, recognition and management of systemic complications of moderate sedation, with particular attention to airway maintenance and support of the respiratory and cardiovascular systems.
ACLS (Advanced Cardiac Life Support)
You will need to be ACLS certified to get your sedation permit from the California Dental Board. Your ACLS certification begins online at www.ACLS-Algorithms.com. ACLS-Algorithms has been providing online ACLS training since 2010 to help health care providers prepare for and pass the American Heart Association ACLS Certification. Your ACLS preparation should not have to be stressful or difficult. ACLS-Algorithms will provide you low-stress, highly focused and an entertaining environment for learning. Begin this online training ASAP after course registration.
CANCELLATION AND REFUND POLICY
Bay Area IV Wisdom reserves the right to cancel or postpone any course, modify the announced course content, limit enrollment, or change the location, time, date or speakers due to unforeseen circumstances. Examples of circumstances that could cause course cancellation include weather, natural disasters, medical conditions, government regulations, facility availability and dental board restrictions. Bay Area IV Wisdom is not responsible for the refund of any part of the course fee as a result of emergencies, unforeseen circumstances or events beyond its control. In the event that Bay Area IV Wisdom cancels a program, the registrant will receive a refund or credit toward another continuing education course.
Full refunds will be made when a course is canceled and is no longer available or a dentist provides documentation proving that they no longer practice dentistry. Bay Area IV Wisdom liability is for the course fee only and is not responsible for any other costs incurred.
Attendees who have registered for a course and are unable to attend will receive credit for the next available course provided their space can be filled. Bay Area IV Wisdom courses may include controversial materials. Bay Area IV Wisdom acknowledges that there is a potential risk to participants using limited knowledge when incorporating new techniques and procedures into their practices.
